Alterações na cavidade oral e as novas modalidades radioterápicas: uma nova perspectiva

Abstract

O câncer de cabeça e pescoço ocupa o quinto lugar na lista de tumores malignos no mundo e pode acometer vários sítios, entretanto a cavidade bucal é o local de maior incidência. Como uma das formas de tratamento temos a radioterapia, que pode ser utilizada isoladamente ou concomitante a quimioterapia. A radioterapia é um método que faz o uso de radiação ionizante para destruir as células tumorais e dispõem de várias modalidades de tratamento, sendo elas: radioterapia bidimensional (2D), radioterapia tridimensional conformada (3D), radioterapia de intensidade modulada (IMRT) e radioterapia guiada por imagem (IGRT). Apesar de ser amplamente utilizada, a radioterapia também é responsável por algumas reações adversas para os pacientes. Dentre as alterações mais comuns associadas a essa técnica na região de cabeça e pescoço podemos citar: xerostomia, mucosite, cáries de radiação, disgeusia, trismo e osteorradionecrose. Todavia, podemos observar que com o grande avanço da tecnologia tais complicações diminuíram ou até mesmo foram prevenidas. Este trabalho tem como objetivo demonstrar por meio de uma revisão de literatura que com o avanço da radioterapia houve uma alteração nas complicações na cavidade bucal decorrentes do tratamento do câncer de cabeça e pescoço. Foi realizado um levantamento através de pesquisas em sites com publicações científicas na área, banco de dados da BIREME, PUBMED, GOOGLE ACADÊMICO e SCIELO. Após a discussão sobre as novas modalidades nos tratamentos de câncer de cabeça e pescoço, conclui-se que as técnicas por meio de IMRT e IGRT demonstraram uma redução nas sequelas que ocorrem na cavidade oral, auxiliando no aumento da sobrevida do paciente.

Head and neck cancer rank fifth in the list of malignant tumors in the world and can affect several sites, however the buccal cavity is the site of greatest incidence. As one of the forms of treatment we have radiotherapy, which can be used alone or concomitantly with chemotherapy. Radiotherapy is a method that uses ionizing radiation to destroy tumor cells and has several treatment modalities: two-dimensional radiotherapy (2D), three dimensional conformal radiotherapy (3D), intensity modulated radiotherapy (IMRT) and radiotherapy image-driven (IGRT). Although widely used, radiotherapy is also responsible for some adverse reactions to patients. Among the most common changes associated with this technique in the head and neck region we can mention: xerostomia, mucositis, radiation caries, dysgeusia, trismus and osteoradionecrosis. However, we can observe that with the great advance of technology such complications have been diminished or even prevented. This study aims to demonstrate through a literature review that with the advancement of radiotherapy there was a change in complications in the oral cavity resulting from the treatment of head and neck cancer. A survey was carried out through research on sites with scientific publications in the area, database of BIREME, PUBMED, GOOGLE ACADEMIC and SCIELO. After discussion of the new modalities in the treatment of head and neck cancer, it was concluded that the techniques through IMRT and IGRT demonstrated a reduction in the sequelae that occur in the oral cavity, helping to increase the patient's survival
